Epidural anesthesia with sacrococcygeal approach in a German Spitz dog for placement of a trochlea prosthesis in the pelvic limb

Abstract

Epidural anesthesia is a type of locoregional anesthetic technique used to assist in trans- and postoperative analgesia, blocking surgical nociception directly in the nerve roots that leave the spinal cord using local anesthetics. The existing approaches to this technique are thoracolumbar, lumbosacral and sacrococcygeal, the latter showing greater safety due to the absence of the medullary cone and dural sac. With this, the present report aimed to demonstrate the epidural technique through the sacrococcygeal approach as a safer option for small dogs such as, in this case, a German spitz. The patient underwent implantation of a trochlear prosthesis in the right pelvic limb due to erosion of the trochlear rhym, and the technique chosen for locoregional blockade was sacrococcygeal epidural anesthesia with the use of lidocaine 0.25 ml/kg and morphine 0.1 mg/kg. No analgesic rescue was necessary during the surgical procedure, as well as in the postoperative period, with only dipyrone 25mg/kg and Meloxicam 0.1 mg/kg being used before it was finished. The patient’s awakening was calm and without signs of pain by the Glasgow scale, concluding that the sacrococcygeal approach ensures effects as satisfactory as the lumbosacral approach, and with less chance of complications due to anatomy.
