This study aimed to report the anesthetic management of a feline undergoing cystotomy using a protocol based on dexmedetomidine, remifentanil, propofol, and isoflurane. A 5-year-old, 4.7-kg neutered male Brazilian Shorthair cat presented with prostration, anorexia, hematuria, and urinary incontinence. Laboratory and ultrasonographic examinations revealed emphysematous cystitis, vesical calculi, and mild nephropathy, indicating the need for surgical intervention. Premedication consisted of methadone (0.2 mg/kg, IM) and dexmedetomidine (3 µg/kg, IM). Anesthesia was induced with propofol (4 mg/kg, IV) and maintained with isoflurane in 100% oxygen, combined with continuous remifentanil infusion (20 µg/kg/h, IV). During surgery, only mild bradycardia was observed, as expected from dexmedetomidine administration, with no hypotension. Four urinary calculi were successfully removed, and the patient exhibited smooth anesthetic recovery. The protocol proved effective in providing analgesia, hemodynamic stability, and satisfactory recovery, highlighting the importance of individualized anesthetic planning in felines undergoing lower urinary tract surgery.
