Abstract

Pyometra is a reproductive disease with a high mortality rate due to the risk of sepsis and multiple organ failure when not diagnosed and treated early with an ovariohysterectomy (OSH). This case report describes the occurrence of the disease in a 4-year-old female Cavalier King Charles Spaniel, which presented with clinical signs including anorexia and apathy. Physical examination revealed pale mucous membranes, moderate dehydration, normal cardiorespiratory parameters (heart rate: 165 bpm; respiratory rate: 25 rpm), body temperature of 39°C, and a distended, sore abdomen upon palpation and percussion. A small amount of brownish, serous discharge was noted in the vaginal region. Diagnosis was confirmed through laboratory and imaging tests. Blood samples were collected for a complete blood count and biochemical profile, which revealed leukocytosis with a left shift, regenerative hypochromic normocytic anemia, and elevated urea and creatinine levels. A vaginal swab was also taken for cytology, showing diestrus and bacterial infection, with abundant neutrophils present. Abdominal ultrasonography demonstrated a large volume of anechoic intrauterine content and thickened uterine walls, consistent with pyometra. Pyometra is the most prevalent reproductive disorder in female dogs and often presents as an emergency due to its potential progression to sepsis, multiorgan failure, and death. The gold standard treatment is OSH, which may be performed electively depending on the severity of the inflammation or infection. However, in select cases, conservative medical treatment may be considered as an alternative approach.
