Feline infectious peritonitis (FIP) is a systemic viral disease with high lethality whose ante mortem diagnosis remains challenging due to the non-specific nature of early clinical signs. This paper reports the case of a one-year-old mixed-breed female cat showing apathy, persistent fever, and abdominal pain. Ultrasonographic examination revealed the presence of a fecaloma, which guided the initial clinical management. After stabilization and resolution of the gastrointestinal condition, the patient suddenly progressed to death, presenting pleural effusion and oral and nasal hemorrhages. Diagnostic confirmation of the effusive form of FIP was achieved through a positive polymerase chain reaction (PCR) test for FCoV in the pleural fluid and was subsequently confirmed by necropsy performed using Virchow’s technique, with macroscopic findings consistent with the disease. The findings included granulomatous pleuritis, pulmonary pyogranulomas, and fibrin deposition on the intestinal serosa. The present study reports a case of feline infectious peritonitis managed at a veterinary clinic, highlighting the clinical signs, the diagnostic methods employed, as well as the limitations encountered during the case investigation and the progression of the condition.
