Modified tibial plateau leveling osteotomy in a dog

Patellar luxation (PL) is one of the most common orthopedic conditions encountered in small animal surgical practice. It consists of the displacement of the patella from its normal anatomical position within the femoral trochlea, which may occur medially or laterally and may be of congenital, developmental, or traumatic origin. Small-breed dogs are more predisposed to this condition. Likewise, cranial cruciate ligament disease (CrCLD) is a frequent orthopedic disorder in the surgical routine of small animals and is characterized by insufficiency of the cranial cruciate ligament (CrCL), which under physiological conditions functions as the primary stabilizing component of the stifle joint. CrCLD is multifactorial and may affect dogs of any breed or age. PL alters stifle biomechanics, overloading the CrCL and potentially leading to its rupture and consequent joint instability. Modified tibial plateau leveling osteotomy (TPLO-m) is presented as a surgical treatment option for addressing both conditions simultaneously. The present report describes a case of PL associated with CrCLD in the right pelvic limb of a 4-year-old male German Spitz that underwent TPLO-m for reduction of the tibial plateau angle (TPA) and alignment of the quadriceps extensor mechanism. It is concluded that TPLO-m is a fundamental surgical technique for promoting early functional limb recovery and pain-free range of motion, resulting in a satisfactory postoperative outcome.
