Abstract

Portosystemic shunt (PSS) is a hepatic vascular anomaly characterized by an abnormal communication between the portal and systemic circulations. Consequently, blood from the abdominal organs bypasses the liver, preventing the normal metabolism of toxic substances and leading to their accumulation in the systemic circulation. This report describes the therapeutic management and surgical treatment of a 2-year-old, 12.1 kg, male mixed-breed dog presenting with asthenia, progressive weight loss, and neurological dysfunction resulting in an inability to perform normal daily activities. Physical examination revealed reduced proprioceptive reflexes in the thoracic and pelvic limbs, disorientation, partial blindness, hyphema, agitation, and trigeminal nerve paresis. Diagnostic investigations included a complete blood count, serum biochemistry with bile acid determination, urinalysis, 4DX® testing, abdominal ultrasonography, and contrast-enhanced computed tomography. Based on the clinical presentation and complementary diagnostic findings, a diagnosis of PSS was established. Medical management was initiated immediately to stabilize the patient before surgical intervention. Following clinical improvement, the patient underwent surgical attenuation of the anomalous vessel using the cellophane banding technique, the definitive treatment for PSS. At the three-month postoperative follow-up, clinical evaluation and laboratory findings were within normal limits. The pathological changes associated with PSS result from diversion of portal blood flow away from the liver, leading to the accumulation of neurotoxic substances and the development of neurological manifestations. This case highlights the importance of combining a thorough clinical examination with appropriate laboratory and imaging studies for accurate diagnosis. Furthermore, preoperative medical management allowed clinical stabilization, enabling safe surgical correction and contributing to a favorable outcome.
