Abstract

Mammary tumors are among the most common neoplastic diseases in cats and are associated with a high rate of malignancy. Extensive surgical procedures, such as mastectomy, represent a significant anesthetic challenge due to intense nociceptive stimulation and extensive tissue manipulation. This study reports the anesthetic management of a cat undergoing unilateral mastectomy for mammary carcinoma, highlighting the use of multimodal anesthesia associated with the tumescent technique as a locoregional analgesic strategy. A 10-year-old mixed-breed female cat presenting multiple nodules in the left mammary chain was evaluated. Premedication consisted of methadone (0.2 mg/kg), dexmedetomidine (4 µg/kg), and ketamine (1.5 mg/kg). Anesthetic induction was performed with propofol to effect, associated with fentanyl (2.5 µg/kg), followed by orotracheal intubation. Anesthesia was maintained with isoflurane in oxygen and continuous infusion of fentanyl (5 µg/kg/h) and ketamine (1 mg/kg/h). Tumescent local anesthesia was performed along the mammary chain to be removed. Histopathological evaluation revealed grade III tubule papillary carcinoma with macrometastasis in the regional lymph node. The combination of general anesthesia, continuous analgesic infusion, and locoregional blockade provided adequate anesthetic stability and intraoperative analgesia. Multimodal anesthesia associated with the tumescent technique appears to be an effective approach for extensive oncologic procedures in felines.
