Abstract

This case report describes the clinical and surgical management of a senior, mixed-breed, female dog diagnosed with chronic proliferative otitis externa associated with apocrine cystomatosis and systemic lupus erythematosus. The patient had a long-standing history of atopic dermatitis with progressive auricular deterioration, unresponsive to previously administered topical and systemic therapies. Due to the lack of therapeutic response, a total ear canal ablation with lateral bulla osteotomy (TECA-BO) was performed, combined with a personalized hypoallergenic natural diet. Histopathological examination confirmed benign apocrine gland proliferation, marked fibrosis, and chondromalacia, consistent with chronic inflammation. The patient exhibited satisfactory postoperative recovery and favorable clinical progression with no recurrence of otic infections, and noticeable improvement in dermatological and behavioral signs. Clinical stabilization was attributed to early surgical intervention, highlighting the importance of integrated and individualized strategies for geriatric dogs with multiple comorbidities. This case underscores the relevance of early diagnosis, continuous clinical monitoring, and a multidisciplinary approach in managing complex dermatological and otological conditions in veterinary practice.
